1. HOW TO ENTER: No purchase necessary. Enter in one of two ways: 1) go to www.wgntv.com , click on the Countdown to Summer logo and follow the instructions to complete the online entry form and attach a full length digital photo of the entrant in jpg format; or 2) mail your entry and full length individual photo at least 3 x 5 in size to:

Spring into Fitness Challenge

Entries must include one full-length photo of only the entrant and an essay telling us, in 200 words or less, why you want to improve your fitness level and some weight along the way. Entrants may only use recent photographs taken no longer than one week before entry submission. Entries must also include the name, age, current body weight, height, address, daytime telephone number, and fitness goals of the entrant. Photos will be assessed and taken into consideration along with the content of the essay. Limit one entry per person. Multiple entries from the same e-mail address will delete earlier entries submitted. All entries become the property of Contest Sponsors (defined below), and none will be returned. Incomplete entries and entries not submitted to Contest Sponsors in one of the two ways specified above will be disqualified. Contest Sponsors are not responsible for entries that are lost, late, garbled, damaged, illegible, rejected or misdirected as a result of faulty internet connections, unavailability of servers, other technical malfunctions, or U.S. postal difficulties. Contest Sponsors will disqualify any entry that fails to comply with these rules. By entering, contestants warrant that their entry and essay are their own original work, are truthful in all respects, and that they own or otherwise have the unconditional right to submit their photo for use and broadcast by the Sponsors as provided in these Rules without limitations or restrictions or payment of any kind.
2. ENTRY DEADLINE: Entries must be received between Monday, March 29, 2010 to Thursday, April 8, 2010 at midnight.
3. WINNER SELECTION AND NOTIFICATION:
a. Finalist Selection: A panel of WGN-TV judges will review all eligible entries and select four (4) finalists based upon essay originality (50%), positive attitude (25%), and photogenic/on-screen appeal (25%). The four (4) contestants will be notified by telephone on or around Monday, April 12, 2010. The contestants must meet all eligibility requirements in order to participate and submit all ‘Countdown to Summer’ medical forms. These are to be completed by their physicians and signed waivers/releases must be submitted before participating in the Countdown to Summer fitness challenge. Contestants must be available in person on Monday, April 19, 2010 at Bally Total Fitness, 1455 W. Webster Ave, Chicago, IL 60614, Monday, May 24, 2010 at WGN-TV, 2501 W. Bradley Place, Chicago, IL 60618, and Monday, May 31, 2010 at Bally Total Fitness. Contestants must also be available via phone on the WGN Morning News on potentially any of the following Mondays (April 26, May 3, May 10, and May 17) or the contestant will be disqualified and an alternate finalist will be chosen. By participating in the Countdown to Summer fitness challenge, contestants agree to be filmed and/or photographed before, during and after their fitness challenge. Contestants agree to be weighed and their body fat measured by Bally personnel before, during and after the Fitness Challenge.
b. Spring Fitness Challenge Contestants: On Monday, April 19, 2010, each of the four contestants will be weighed and their body fat measured on WGN Morning News by Bally personnel at a Bally Total Fitness location. The four contestants will participate in a 6-week fitness challenge from Monday, April 19, 2010 through Monday, May 31, 2010. This includes attending weekly workout sessions with a personal trainer (two (2) one-hour individual sessions per week for 6 weeks), receiving and following nutrition counseling from Dr. Ian Smith, and working out at Bally Total Fitness using the One (1) Year Premier membership to Bally Total Fitness. On Monday, May 24, 2010 all four (4) contestants will appear on the WGN Morning News in studio. If a contestant should be disqualified or otherwise drop out for any reason, Sponsors reserve the right (but are not obligated) to select an alternate individual to participate, if time permits.
c. Finale: On Monday, May 31, 2010, each individual will appear on the WGN Morning News live at a Bally Total Fitness location. Each contestant will be weighed, their body fat measured and side-by-side shots taken by Bally personnel during the newscast.
4. PRIZES: The Contestant’s Prize consists of twelve (12) personal training sessions per contestant (two (2) per week for (six) 6 weeks) (ARV $600) and a one (1) year National Access Non-Renewable membership to Bally Total Fitness. Membership is good at participating Bally Total Fitness brand health clubs in the United States. Contestant prize winner may not be an existing Bally Total Fitness member. Contestant prize winner must be 21 years of age or older. Contestant prize winner must execute a no-charge membership contract and a waiver and release. Membership is non-renewable and cannot be sold, reproduced or redeemed for cash. Bally Total Fitness club rules and regulations apply to any holder of such Membership. Additional charges will apply for some services, including but not limited to Personal Training, Specialty Classes and Kids Club. Membership is transferable, and transferee is subject to all foregoing conditions. Prize must be redeemed within six (6) months after award to winner defined as final day of promotion, May 31, 2010. (ARV $450). Entrants understand that Sponsors are not liable for injuries, losses, or damages of any kind arising from participation in this promotion and acceptance, possession, and use of prize.
5. ELIGIBILITY RESTRICTIONS:
a. This contest is void where prohibited or restricted by law. Employees of WGN-TV, Bally Total Fitness Corporation, Dr. Ian Smith ("Contest Sponsors") and their parent companies, subsidiaries, affiliates and advertising agencies, and their respective immediate family members, are not eligible to win.
b. Entrants must be men or women 21 years of age or older and a US citizen and legal resident of the Chicagoland DMA viewing area defined as: Cook, DeKalb, DuPage, Grundy, Kane, Kankakee, Kendall, LaSalle, Lake, McHenry and Will Counties in Illinois, and Jasper, LaPorte, Lake, Newton and Porter Counties in Indiana.
c. Each Contestant member must provide prior to starting the WGN Morning News Countdown to Summer on Monday, April 19 2010: (i) the completed medical forms signed by a physician by noon on Friday April 16, 2010 stating that he/she is physically able to participate in this contest; and (ii) signed release/waivers provided by Sponsors. A contestant’s failure to provide the physician’s report and signed release/waivers by that date will result in the disqualification of the contestant and the selection of an alternate contestant.
6. CONDITIONS OF ENTRY:
a. Use of Entrant's Name and Info: By entering, each entrant grants permission, without further compensation, to Contest Sponsors and their agents and licensees, to use the entrant’s name, voice, photograph, likeness, appearance, biographical material, and entry submissions (including essay, weight specifics and group photo) in any media for broadcast and promotional purposes in connection with the contest.
b. Execution of Affidavit and Release: By entering, each entrant agrees that if he or she is a finalist, he/she will sign a release and waiver of liability.
c. Release of Sponsors of Liability: By entering, each entrant agrees to release Contest Sponsors, their parent companies, subsidiaries, affiliates, shareholders, officers, directors, employees, and agents from any claim, loss, liability, damage or injury of any kind arising from the entrant’s participation in the contest, receipt or use of the contest prize, and any other contest-related travel or activity.
d. No Prize Substitutions: No substitute prizes or cash redemptions are available, except at Contest Sponsors’ sole discretion.
7. INTERNET/FORCE MAJEURE: If for any reason, this contest is not capable of running as planned, including infection by computer virus, tampering, fraud, technical failure, or any other cause which corrupts or threatens the administration, security or integrity of the contest, Contest Sponsors reserve the right, in their sole discretion, to terminate or suspend the Contest or any portion of the Contest. ANY ATTEMPT TO DAMAGE OR UNDERMINE THE FAIR AND LEGITIMATE OPERATION OF THIS CONTEST WILL RESULT IN DISQUALIFICATION FROM THE CONTEST. Information submitted in entries, including entrant's name, address, e-mail address, etc, shall become the property of Contest Sponsors and Prize Provider, and may be used by them for marketing or promotional purposes. Contest shall be governed and enforced pursuant to Illinois law, excluding choice of law provisions.
